Think about the paid and unpaid supports you need to achieve your goals. What are your preferences for how and when you would like to receive those supports?
Your care team will complete an assessment and work with you to identify what kinds of supports you need and what types of services will best provide that support to meet your identified long-term care outcomes.
You can decide if you want others to be a part of this process such as your family or friends.
